The Supporting Aboriginal Seniors at Home (SASH)
The SASH Team strives to provide improved access to primary care and chronic disease management services. These services are available to Aboriginal Seniors aged 55 and over, within the London area both urban and reserved based. Our goals are to network with community services while providing culturally appropriate care to enable the seniors to live safely in their homes.
SERVICES PROVIDED:
NUNNURSE PRACTITIONER
- The Nurse Practitioner will provide primary care and chronic disease management services
- Visits will occur in both the clinic and the home setting, based on the needs and abilities of the client
- Services will be available to existing primary care clients of SOAHAC as well as new clients who do not currently have a primary care provider
SENIOR’S HEALTH ADVOCATE
The Senior's Health Advocate will assist those in need accessing, community and social services.
The Senior’s Health Advocate will: - Develop client centered health
maintenance plans related to seniors care - Provide health related information and support as required
- Ensure continuity of care by communicating with the clients primary care provider and other service providers as required
- Advocate on behalf of the client to
ensure appropriate services are being accessed and needs are being met - Make referrals to outside agencies as needed
PATIENT NAVIGATOR
The Patient Navigator (RN) will help clients find their way through the complex hospital system by: - Helping the health care team identify Aboriginal clients and families in need of the service.
- Working with the health care team to develop a client-centric plan of care, by incorporating traditional healing and culturally safe practices based on client choice
- Supporting clients, caregivers and families during communication with physicians, nurses and members of the health care team
- Arranging language and cultural translation services when needed
- Helping connect clients and families with Traditional Aboriginal Healers
- Counselling clients and their families on understanding the plan of care while in hospital
- Assisting with discharge planning

- Bring your health card and certificate of Indian status (if applicable) on your first visit.
- Bring a list of your medications and glucose-recording book with you if you have them.
- If you have an urgent medical emergency, go to a Hospital Emergency Room and tell them the name of your Doctor or Nurse Practitioner at SOAHAC.
